Links between Cardiovascular disease, Lung disease, and Obstructive sleep apnea in HIV: Understanding complex patients

Role: Co-PI with Heidi Crane and Joseph Delaney
People with HIV (PWH) face increased morbidity and mortality from non-infectious heart, lung, blood, and sleep (HLBS) disorders. There are many unanswered questions in the current era, including the role of emerging risk factors such as COVID-19, newer antiretroviral therapy (ART) medications, non-traditional risk factors such as social determinants of health, and the increasingly complex interactions of HLBS conditions with each other.
This proposal builds on the large, diverse Center for AIDS Research (CFAR) Network of Integrated Clinical Systems (CNICS) cohort of PWH with comprehensive clinical data, specimens, and socio-behavioral measures to study understudied necessary HLBS conditions such as obstructive sleep apnea, as well as to study evolving risk factors for other conditions such as myocardial infarction, stroke, atrial fibrillation, and venous thromboembolism.
New and timely knowledge about the increasing complexity of HLBS conditions among PWH, including screening, risk factors, and complex interactions of HLBS conditions with each other and other syndemic conditions, will inform clinical decision-making and improve understanding of the mechanisms that increase the frequency of HLBS diseases and risk of poor outcomes in PWH in current clinical care.

Chronic Lung Disease and COVID-19: Understanding Severity, Recovery and Rehabilitation Needs (LAUREL Study)

Coronavirus-19 disease (COVID-19) is a global pandemic, and Veterans are at risk of infection and poor outcomes from COVID-19. However, the long-term effects of COVID-19 have yet to be characterized or quantified.
We will determine the outcomes of patients after COVID-19, including their trajectory of health recovery and rehabilitation needs.
We will evaluate risk factors for worse long-term outcomes from COVID-19, such as chronic lung disease and other chronic conditions, to identify individuals at the most significant risk of doing poorly and needing rehabilitation services.
We will assess the feasibility and acceptability of a virtually-delivered, home-based personalized telerehabilitation program for survivors of COVID-19.
Our pilot study will inform future more extensive trials of this novel program to determine if it can improve the function and well-being of individuals after COVID-19.

VAPSHCS Program Development in Precision Oncology for Screening and Management of Lung Cancer (LPOP)

This program aims to increase delivery and access to precision oncology to improve the early detection and management of lung cancer and patient outcomes.
We will utilize an integrated practice unit (IPU) model to promote high-quality, high-value patient-centered lung cancer care that we will leverage to support an innovative and efficient portfolio of research studies encompassing personalized, precision oncology approaches to lung cancer screening (LCS) and lung cancer treatment.

Dexamethasone in hospitalised COVID-19 patients not on intensive respiratory support

Improving the Reach and Effectiveness of Smoking Cessation Services Targeted to Veterans Living with HIV

People living with HIV are a priority group for targeting smoking cessation. Still, standard care cessation treatments reach or benefit only a tiny proportion of these individuals. Interventions are needed to engage smokers who are not yet ready to quit or seek treatment, as well as assist those ready to quit smoking.
This project will compare the effectiveness of an HIV-specific comprehensive wellness intervention designed to appeal broadly to smokers living with HIV to the standard of care services, including referral to the National VA Quitline and SmokefreeVET texting program.
Participants in both study arms will have access to pharmacotherapy available through their Veterans Affairs (VA) healthcare providers.

AdvanCing High quality COPD care for people with immune dysfunction by implementing Evidence-based management through proactive E-consults (ACHIEVE)

Chronic obstructive pulmonary disease (COPD) is common and associated with morbidity and mortality in HIV-infected patients. However, COPD is often underdiagnosed, and care is suboptimal.
We propose to test an intervention to proactively support primary care providers of HIV-infected patients and make evidence-based recommendations to improve the care and health of their patients with COPD.

Characterizing chronic lung disease in youth living with HIV: quantitative chest CT analyses

As increasing numbers of children living with HIV survive to adolescence and early adulthood, chronic lung disease is emerging as an important but poorly understood complication among youth living with HIV.
This project determines the clinical characteristics associated with lung abnormalities defined by quantitative CT metrics in youth living with HIV. It determines whether these quantitative CT metrics can identify chronic lung disease subtypes related to lung function growth trajectories.
Findings from this study will fill gaps in knowledge of chronic lung disease subtypes in youth living with HIV, provide a better understanding of mechanisms earlier in the cascade of impairment caused by chronic lung disease in these youth, and offer an opportunity for early subtype-targeted interventions to mitigate disease progression.
